I think the issue is that Monk's Token Bar may not call the portions of the API that MIDI QOL is listening for. I had a note that they did not play well together.

The best thing I can think of is under Workflow Settings you can set "Prompt Players to Roll Saves". You could set that to "Chat Message", and then your players could just ignore the prompt if they shouldn't be affected by the spell. Then it would roll all the NPCs automatically but not the players.
